The plaintiff is suing the defendant for breach of fiduciary duty, negligence, gross negligence, breach of express contract, and breach of express contract.  The plaintiff alleges that an employee of his firm, ACT Media LLC, went to the defendant’s branch at Patterson and Willow Lawn avenues and opened a business bank account. However, the employee did not have the plaintiff’s social security number and other confidential information, which bank representatives gave to the employee. This former employee also opened other accounts at other financial institutions, using the information the defendant’s allegedly supplied and “unilaterally began engaging in a series of unauthorized bank transactions between several entities and herself.” The plaintiff is seeking $1 million judgment with punitive damages of $350,000.

The plaintiff is suing the nursing facility for negligence and gross negligence after a metal clip that was being used on the plaintiff’s bed struck an exposed outlet and sparked.  The plaintiff alleges that Overby, a nurse, witnessed the sparking and left the room.  Within minutes, more sparks ignited the bedspread and caused severe leg burns to the plaintiff. The plaintiff is seeking $10 million for each count of negligence and $350,000 in punitive damages.
